There are normally only three reasons you would need to add an oasis from the orders tab:

• Transfer without a visit
• Transfer Discharge without a visit
• Discharge without a visit

When transitioning over to Careficient, the recert oasis will need to be added in this way.  Click here for more information on <udl-blue>Transition Patients (Home Health)<udl-blue></udl-blue></udl-blue> or <udl-blue>Transition Patients (Hospice)<udl-blue></udl-blue></udl-blue>.

TRANSFER WITHOUT A VISIT OASIS

Before you can add a Transfer Oasis, you must <udl-orange>[Place Patient On Hold]<udl-orange>. </udl-orange></udl-orange>

Enter the required information and select <btnw>   Save   <btnw> when finished.</btnw></btnw>

To add an oasis, select the drop down <icon-action>xxx<icon-action> to the of the 485 and choose <icon-documentadd>xxx<icon-documentadd><udl-orange>Add Oasis<udl-orange> </udl-orange></udl-orange></icon-documentadd></icon-documentadd></icon-action></icon-action>

Choose "◉ 6 - Transferred to an inpatient facility - patient not discharged from agency" and select <btnw>    Save   <btnw>. The OASIS will open and you will be able to fill it out. Click here to learn more about </btnw></btnw><udl-blue>Navigating The Oasis<udl-blue></udl-blue></udl-blue>

TRANSFER DISCHARGE WITHOUT A VISIT OASIS

Before you can add a transfer discharge oasis, you must first discharge the patient. Select the <udl-orange>[Discharge Patient]<udl-orange> Link.</udl-orange></udl-orange>

Enter the required information and select <btnw>   Save   <btnw> when finished. </btnw></btnw>

NOTE: For Careficient Agency Administrators, “Discharge Reason Type” options can be set in Settings > Lookup Tables > Patient Support Tables > Discharge Reason Type.

Select the drop down <icon-action>xxx<icon-action> to the of the 485 and choose <icon-documentadd>xxx<icon-documentadd><udl-orange>Add Oasis<udl-orange>. </udl-orange></udl-orange></icon-documentadd></icon-documentadd></icon-action></icon-action>

Select "◉ 7 - Transferrred to an inpatient facility - patient discharge from agency" and then select <btnw>    Save   <btnw>. The OASIS will open to be filled out. </btnw></btnw>

DISCHARGE WITHOUT A VISIT OASIS

Before you can add a Discharge oasis, you must first discharge the patient. Select the <udl-orange>[Discharge Patient]<udl-orange> Link.</udl-orange></udl-orange>

Enter the required information and select <btnw>   Save   <btnw> when finished. </btnw></btnw>

Select either "◉ 8 - Death at home" OR

"◉ 9 - Discharge from Agency". Select <btnw>    Save   <btnw> when you are finished. The OASIS will open up to be filled out. </btnw></btnw>
