This option provides you with a list of Patients that have a Clinical Update interim order. If you are NOT using the Agency rule that reads: Process To Connect Care Needs To 485 / CUP, you will only see the following information:

The rest of the columns will be blank.

If you activate the agency rule that reads: <udl-blue>Process To Connect Care Needs To 485 / CUP<udl-blue>, additional information displays in the rest of the columns for Clinical Updates that you are using to document your Case Conference/Meetings.

You can use this option to manage your Case Conference/Meetings and easily update Care Needs, Goals and Interventions as a result of the Case Conference/Meeting outcome for each patient.

If you have already setup your signature in Careficient, you will have the option to change it before signing orders if you choose the <udl-orange>Change my Signature<udl-orange> link.

SIGN ALL CASE CONFERENCE ITEMS <returntomenu>menu↑<returntomenu>

If you are providing Case Conferences/Team Meetings on a standardized scheduled (i.e., every week, every 2 weeks, etc.) and are collecting meeting attendee signatures, you can use the Clinical Update (CUP) interim order as your meeting's documentation and have Associates, who participated in the Patient's chart review, sign each Case Conference interim order. Each document can be signed individually and can be signed "in mass". Associate must be part of the clinical team in order to sign the Case Conferences.

NOTE: When creating CUP orders, if you are using them for Case Conferences, make sure that you check the box within the CUP order indicating it is a "Case Conference".

Check one/multiple items listed that you are signing and select the <btn>   Save   <btn> button

All CUP orders will populate here regardless of whether they are for Case Conference or just a normal Clinical Update Order.  Select the drop down <btn>   Louisiana Labour of Love       ▾   <btn> to the right of Agency to select your agency(ies).

You can search by patient first or last name...

or by effective dates from and to.

Click <btnw>   Search   <btnw> when you have made all your changes.

Clicking on the "Patient Name" <udl-orange>McNaughton, James<udl-orange> link, will take you to the patient's encounter (chart).

The drop down <icon-dropdown>xxx<icon-dropdown>to the right of the CUP Status, will give you information regarding the status changes of the order.

If you hover over the address book <icon-addressgray>xxx<icon-addressgray> icon you will get a pop-up with the physician's contact information.

EDIT CUP ORDER <returntomenu>menu ↑<returntomenu>

Clicking on the edit pencil <icon-editpencil>xxx<icon-editpencil> icon to the right of the physician's order, will allow you to edit the order just like you normally would from the Orders Tab.  

We recommend including specifically the discussion during the Case Conference as well as those in attendance.

PRINT CUP <returntomenu>menu ↑<returntomenu>

You can print the CUP Order as well by clicking on the printer <icon-printer>xxx<icon-printer> icon.

To the right of the order you have several drop down options.  Click on the drop down arrow <icon-action>xxx<icon-action> icon.

ATTACH CARE NEEDS <returntomenu>menu ↑<returntomenu>

You will notice that there are Cardiovascular and General Care Needs added already but you can add additional Care Needs by clicking on "<icon-greenplussign>xxx<icon-greenplussign> <udl-orange>Attach Care Needs<udl-orange>".   You can change the effective date of the order, or if you have had a ROC or Follow-up since the last CUP Order, you can pull the Locator items to this CUP Order from that service note by choosing "Apply Service Note Data" and choosing the specific service note.

FAX TO PHYSICIAN <returntomenu>menu ↑<returntomenu>

If the order is in an approved status, you will have the option to <icon-fax>xxx<icon-fax> <udl-orange>Fax To Physician<udl-orange>.

DESKTOP FAX TO ANYONE <returntomenu>menu ↑<returntomenu>

If your agency has <icon-fax>xxx<icon-fax> <udl-orange>Desktop Fax to Anyone<udl-orange> enabled, you will be able to fax to others.

PRINT AND UPDATE AS SENT <returntomenu>menu ↑<returntomenu>

You can also <icon-printercheck>xxx<icon-printercheck> <udl-orange>Print And Update As Sent<udl-orange> for a hard copy.

APPLY MY SIGNATURE <returntomenu>menu ↑<returntomenu> 

If the CUP is in an approved status, you can <icon-editpencil>xxx<icon-editpencil> <udl-orange>Apply My Signature<udl-orange>

You will be prompted to enter your signature. Select <btnw>    Accept    <btnw> to continue.

REMOVE MY SIGNATURE <returntomenu>menu ↑<returntomenu>

Once a signature is applied, if you need to remove it, select <icon-trashcan>xxx<icon-trashcan> <udl-orange>Remove My Signature<udl-orange>

EMAIL PHYSICIAN TO APPROVE <returntomenu>menu ↑<returntomenu>

You can <icon-email>xxx<icon-email> <udl-orange>Email Physician To Approve<udl-orange> the order.

EDIT EFFECTIVE DATE <returntomenu>menu ↑<returntomenu>

If the order is in an unapproved status, you can <icon-editpencil>xxx<icon-editpencil> <udl-orange>Edit Effective Date<udl-orange

APPLY SERVICE NOTE DATA <returntomenu>menu ↑<returntomenu>

You can <icon-documentadd>xxx<icon-documentadd> <udl-orange>Apply Service Note Data<udl-orange>

EDIT CARE NEED <returntomenu>menu ↑<returntomenu>

To edit a Care Plan, click on the edit pencil <icon-editpencil>xxx<icon-editpencil> icon to the right of the Care Need that you want to make changes to.

  1. End Date - End the Care Need by entering an end date
  2. Care Need Level - Change the level of the Care Need by selecting the drop down <btn><select>     ▾   <btn> and making your selection.
  3. Other Care Need - If this is a one time Care Need only for your visit, you can enter it here. It does not flow to the following service notes.
  4. Other Goal - If this is a one time Goal only for your visit, you can enter it here. It does not flow to the following service notes.
  5. Other Intervention - If this is a one time Intervention only for your visit, you can enter it here. It does not flow to the following service notes.
  6. Save - Select the Click <btnw>    Save   <btnw> button to confirm your changes.
  7. Cancel - Select the <btnw>  Cancel  <btnw> button to exit without saving changes.

DELETE CARE NEED <returntomenu>menu ↑<returntomenu>

If you have not documented in the service notes against this Care Need, you can delete it by clicking on the Trash Can <icon-trashcan>xxx<icon-trashcan> icon.  If there is documentation in the service note against this Care Need, you will not be able to delete it.

PRINT CUP <returntomenu>menu ↑<returntomenu>

You can print the selected Care Need by clicking on the Printer <icon-printer>xxx<icon-printer> Icon.

There are several ways to update a Care Need.  Click on the drop down <icon-action>xxx<icon-action>i con to the right of the Care Need that you are wanting to make changes to

ADD GOALS <returntomenu>menu ↑<returntomenu>

To add goals, click on the drop down to the right of the Care Need that you want to add goals to and click "<icon-editpencil>xxx<icon-editpencil> <udl-orange>Add Goals<udl-orange>".

  1. Enter Goals Search Term - Enter a specific goal you would like to search for
  2. Search - Select the <btnw>   Search   <btnw> button when you have made your desired choices.
  3. Select All - Select the <icon-blankcheck>xxx<icon-blankcheck> icon to select all entries.
  4. Select Individual - Select the <icon-blankcheck>xxx<icon-blankcheck> icon to select individual entries.
  5. Goal Status Code - Select a progress toward goals option from the drop down.
  6. Other - Enter any other information you would like here but note that this will not show up in service notes.
  7. Start Date - Enter the effective date of the goal. Make sure to add a start date so that the goal doesn't show up in prior service notes.
  8. End Date - Enter the end date for the goal. We do not recommend adding an end date until you are ready to end the goal.
  9. Save - Select the Click <btnw>    Save   <btnw> button to confirm your changes.
  10. Cancel - Select the <btnw>  Cancel  <btnw> button to exit without saving changes.

EDIT GOAL DESCRIPTIONS <returntomenu>menu ↑<returntomenu>

To make the goals more specific to your patient, click on the drop down to the right of the Care Need that you want to edit descriptions for and choose "<icon-editpencil>xxx<icon-editpencil> <udl-orange>Edit Goal Descriptions<udl-orange>".

Click on the goal that you want to edit.

Make the necessary changes and click <btnw>    Save   <btnw>

You will now see the following <icon-paper>xxx<icon-paper> icon.  Hover over this icon to see what the goal was prior to your changes.

A pop-up displays showing the the goal prior to editing.  Click <btnw>    Save   <btnw> when finished.

EDIT GOALS <returntomenu>menu ↑<returntomenu>

To edit current goals, click on the "<icon-editpencil>xxx<icon-editpencil> <udl-orange>Edit Goals<udl-orange>" link.  

Keep in mind that the only editing you can do once they have been added from a prior order, is to end them.  Only newly added goals can be edited to read something different.  Add the end date for the goals you want to end and click "Save" when finished.

ADD INTERVENTIONS <returntomenu>menu ↑<returntomenu>

To add interventions, click on the drop down to the right of the Care Need that you want to add interventions to and click "<icon-editpencil>xxx<icon-editpencil> <udl-orange>Add Interventions<udl-orange>".

  1. Enter Interventions Search - Enter a specific intervention you would like to search for
  2. Search - Select the <btnw>   Search   <btnw> button when you have made your desired choices.
  3. Select All - Select the <icon-blankcheck>xxx<icon-blankcheck> icon to select all entries.
  4. Select Individual - Select the <icon-blankcheck>xxx<icon-blankcheck> icon to select individual entries.
  5. Start Date - Enter the effective date of the interventions. Make sure to add a start date so that the intervention doesn't show up in prior service notes.
  6. Save - Select the Click <btnw>    Save   <btnw> button to confirm your changes.
  7. Cancel - Select the <btnw>  Cancel  <btnw> button to exit without saving changes.

EDIT INTERVENTION DESCRIPTIONS <returntomenu>menu ↑<returntomenu>

To make the interventions more specific to your patient, click on the drop down to the right of the Care Need that you want to edit descriptions for and choose "<icon-editpencil>xxx<icon-editpencil> <udl-orange>Edit Intervention Descriptions<udl-orange>".

Click on the intervention that you want to edit.

Make the necessary changes and click <btnw>    Save   <btnw>

You will now see the following <icon-paper>xxx<icon-paper> icon.  Hover over this icon to see what the intervention was prior to your changes.

A pop-up displays showing the the intervention prior to editing.  Click <btnw>    Save   <btnw> to continue.

APPROVE CUP ORDER <returntomenu>menu ↑<returntomenu>

You will do this for each patient that you need to conference on.  Once you have completed Case Conference, you will click on the Edit Pencil <icon-editpencil>xxx<icon-editpencil> icon to the right of the CUP (Clinical Update) Order.  Make any necessary changes, sign and <btnw>    Approve    <btnw> the order.  

ADD CUP ORDER FOR NEXT CASE CONFERENCE MEETING <returntomenu>menu ↑<returntomenu>

The change you made to the Care Need/Interventions/Goals, will now print with the CUP (Clinical Update) Order.   You will need to make sure and add the next CUP (Clinical Update) Order for your next Case Conference Meeting.  To do so, click on the name of your patient <udl-orange>McNaughton, James<udl-orange>.

Click on the drop down to the right of the 485 and choose "<icon-greenredarrows>xxx<icon-greenredarrows> <udl-orange>Add Interim Order<udl-orange>". Remember if you have an interim order that is not saved, you will not have the option to Add Interim Order.

Select the drop down <btn>   < Select >      ▾   <btn> to the right of Interim Order Type to select your order type.  Choose "Clinical Update".  Enter the date of your next Case Conference and <btnw>    Save   <btnw>

Make sure to check "Yes" to Case Conference/Meeting. Then <btnw>    Save/Close    <btnw> the order.  It will need to stay open until the next case conference meeting.
