Forms Reports are pulled from the patient and associate Documents Tab under Forms. To run a Forms Report, go to Reports > Forms
Select the drop down <btn> < Select > ▾ <btn> to the right of Form Type to select the type of form you want a report on
Select the drop down <btn> Louisiana Labour of Love ▾ <btn> to the right of Agency to select your agency.
Select the drop down <btn> All ▾ <btn> to the right of Agency Type to choose the agency type.
Enter your From To dates
You can filter by selecting the drop down <btn> All ▾ <btn> under Type
OR you can filter by selecting the drop down <btn> All ▾ <btn> under Status
Click <btnw> Search <btnw>
When you have chosen all your options, click on the type of report you want and then click "Export".
