ROLE PRIVILEGES NEEDED: <udl-white>Edit Facility<udl-white> | <udl-white>Delete Facility<udl-white>

The Facilities Contact Info Tab allows you to enter your facilities address/phone information, referral source contacts as well as any ancillary phone information.

ADD CONTACT INFO <returntomenu>menu ↑<returntomenu>

To add contact info, select the <icon-greenplussign>xxx<icon-greenplussign> <udl-orange>Add Contact Info<udl-orange> link.

Enter the required information and select <btnw>   Save   <btnw> when finished. You can indicate the preferred phone by selecting the checkbox to the left of the phone.  You can also enter multiple phone numbers by selecting

<btnw>    Add Phone   <btnw> for each phone number entered.

EDIT ADDRESS/PHONE INFO   <returntomenu>menu ↑<returntomenu>

Select the <udl-orange>[Edit]<udl-orange> link to edit the Address/Phone Info.

Enter the required information and select <btnw>   Save   <btnw> when finished.

DELETE ADDRESS/PHONE INFO <returntomenu>menu ↑<returntomenu>

Select the <udl-orange>[Delete]<udl-orange> link to delete the Address/Phone Info.

You will be asked if you are sure you want to delete.  Select "OK" to confirm.

ADD CONTACT <returntomenu>menu ↑<returntomenu>

To add a Referral Source Contact, select the <icon-greenplussign>xxx<icon-greenplussign> <udl-orange>Add Contact<udl-orange> link.

Enter the required information and select <btnw>   Save   <btnw> when finished

EDIT REFERRAL SOURCE CONTACTS <returntomenu>menu ↑<returntomenu>

Select the <udl-orange>[Edit]<udl-orange> link to edit the Referral Source Contacts

Make the necessary changes and select <btnw>   Save   <btnw> when finished.

DELETE REFERRAL SOURCE CONTACTS <returntomenu>menu ↑<returntomenu> 

Select the <udl-orange>[Delete]<udl-orange> link to delete the Referral Source Contacts.

You will be asked if you are sure you want to delete.  Select "OK" to confirm.

ADD PHONE <returntomenu>menu ↑<returntomenu>

To add an Ancillary Phone, select the <icon-greenplussign>xxx<icon-greenplussign> <udl-orange>Add Phone<udl-orange> link.

Enter the required information and select <btnw>   Save   <btnw> when finished.

EDIT ANCILLARY PHONE INFO <returntomenu>menu ↑<returntomenu>

Select the <udl-orange>[Edit]<udl-orange> link to edit the Ancillary Phone Info.

Make the necessary changes and select <btnw>   Save   <btnw> when finished.

DELETE ANCILLARY PHONE INFO <returntomenu>menu ↑<returntomenu>

Select the <udl-orange>[Delete]<udl-orange> link to delete the Ancillary Phone Info

You will be asked if you are sure you want to delete.  Select "OK" to confirm.
